Trinity Community Health Center, LLC (TCHC) is a Limited Liability Company
registered as a healthcare organization based in Miramar, Florida. TCHC operates by a
group of highly skilled and knowledgeable medical professionals, including Autonomous
Nurse Practitioners and physicians, all with doctorate degrees.
TCHC offers services in several languages, such as English, Spanish, Creole, and French.
These services will range from standard general primary care/ family medicine,
psychiatry and mental health, free HIV testing and STD treatment, work and school
physical examination, counseling and therapy, telemedicine, and especially preventative
health care services with readily available referral specialty services to individuals of all
ages.
We want to make everyone feel like they are part of our grand family!
The organization will provide optimal quality primary care medical services targeting all
people of all age groups. In addition, it will offer high-quality healthcare services to its
customers to win their trust and loyalty. We aim to render care in an environment of love

Our Well-Trained Providers:
Dr. Jean B. Merozier
Dr. Merozier is an autonomous nurse who always provides professional and efficient quality care that exceeds patients' expectations. He started his journey at Florida Atlantic University, where he received a bachelor's in Nursing. Then, he continued his educational goal at Nova South Eastern University and graduated as a Family Nurse Practioner.
Finally, he obtained his doctorate in Psychiatric Mental Health from the University of North Florida. Dr. Merozier has been working for eleven years in our communities. He has been contributing to better health care and patient satisfaction in many healthcare clinics and hospitals such as Jackson Health System, Mount Sinai Medical Center, North Shore Hospital and UNihealth of South Florida.

Dr. Merozier is motivated to help people, especially those in underserved communities. He is always using his skills, knowledge, patience, honesty, humility, and courage, to mix the traditional means of caring with the innovative ways of modern medicine and today's technology for better health outcomes for his patients and their families.
Dr. Moya Martin
Dr. Moya Martin is a board certified Family practitioner based in Palm Beach County for over 20 years. Born and raised in the picturesque Island of Jamaica, she settled on the sunny shores of Florida to attend the University of Miami. After graduating from University of Miami with honors, she commenced her training at the medical program of Nova Southeastern University in Davie, Florida, graduating in the top 10 percent of her class. Upon completion of her graduate studies, she completed a general surgery internship at Mercy Fitzgerald and Mercy Philadelphia Hospital in Pennsylvania. Dr. Martin acquired additional training at Bay Regional Hospital in Michigan, successfully completing her residency in family medicine.

As an experienced and highly trained health professional, Dr. Martin has provided expert medical care to patients from all walks of life in the role of a Primary Care Physician, Hospitalist, and Emergency Room Physician. In addition, she is an accomplished Aesthetic and Weight-loss medical practitioner, having helped countless patients with looking and feeling their best.
Dr. Jean C. Medina
Dr. Medina is an autonomous advanced practice nurse (A-APRN) in Florida who completed a graduate degree in advanced practice nursing and certificate in family care. He went to Miami Dade College to earn his associate degree in Nursing. Then at Flordia Atlantic University for his Bachelor of Science in Nursing with a certification in family practice from Nova Southeastern University and his docorate in Physchiatric/Mental Health Nurse Practioner (DNP-PMHNP) from the University of North Florida. For about ten years, Dr. Medina has performed in Florida in communities with underserved populations with a shortage of PCPs who speak different languages. He worked in pain management, primary care clinics, long-term nursing homes/acute rehab facilities, and hosptals like Hampton Court Nursing & Rehabilitation Comprehensive Health Center (CHC) and HCA Aventure Hospital and Medical Center.
Dr. Medina dedicates his life to the healthcare system to serve the general population with caring and empathy. He is trained to perform the most practicial and modern nursing/medicine in Florida.
